The loading phase consists of 300 mg administered subcutaneously at weeks 0, 1, 2, 3, and 4. This is followed by 300 mg every 4 weeks for maintenance. It's important to note that each dose is typically given as two 150 mg injections.

Plaque Psoriasis in Children

For children with plaque psoriasis, the dosage is based on body weight. Those weighing between 15 kg and less than 50 kg receive 75 mg, while those 50 kg or more receive 150 mg.

Psoriatic Arthritis

The dosage for psoriatic arthritis may be the same as for plaque psoriasis if both conditions are present. However, if only psoriatic arthritis is present, the dosage may differ based on whether the patient also has plaque psoriasis.

Ankylosing Spondylitis

For ankylosing spondylitis, Cosentyx can be prescribed as a loading dose of one 150-mg injection once per week for 5 weeks, followed by 150 mg once every 4 weeks. Alternatively, it can be prescribed as 150-mg injections once every 4 weeks without a loading dose. Intravenous (IV) infusion dosage may be 6 mg per kg of body weight the first week, then 1.75 mg/kg every 4 weeks.

Non-Radiographic Axial Spondyloarthritis

For non-radiographic axial spondyloarthritis, a loading dose of one 150-mg injection per week for 5 weeks may be prescribed, followed by a dosage change to 150 mg once every 4 weeks.

Hidradenitis Suppurativa

For adults with hidradenitis suppurativa, the typical starting dosage is 300 mg once per week for the first 5 weeks, followed by a dosage decrease to 300 mg once every 4 weeks.

Hidradenitis Suppurativa in Children

The recommended dose for children with hidradenitis suppurativa is based on body weight: 75 mg for those between 15 kg and less than 50 kg, and 150 mg for those 50 kg or more.

For enthesitis-related arthritis in children, the recommended dose is based on body weight: 75 mg for those between 15 kg and less than 50 kg, and 150 mg for those 50 kg or more.

Missed Doses

If a Cosentyx dose is missed, it should be administered as soon as remembered, but not doubled up on the next dose. For IV infusions, missed appointments should be rescheduled as soon as possible.

Various Forms and Precautions

Cosentyx comes in various forms for subcutaneous injection: single-use Sensoready pen, single-use UnoReady pen, and single-use prefilled syringe. It also comes as a solution in a single-use vial for IV infusion, which is administered only by a healthcare professional.

If you have a latex allergy, the Sensoready pen and the prefilled syringe have caps that contain natural rubber.

Long-Term Use

Cosentyx is meant for long-term use if it is determined to be safe and effective for the patient.
